چکیده
Medicare Coverage of Anesthesia Services During Screening Colonoscopies for Patients at LowRisk of Sedation-Related Complications In 2014, theCenters forMedicare&Medicaid Serviceswaived patient cost sharing for anesthesia services during screening colonoscopies.1 The current professional guidelines recommend that sedation be provided by the gastroenterologist-nurse team; a separate anesthesiologist or nurseanesthetist should be involved and paid separately only for patients with an increased risk of sedationrelated complications.2 The stated rationale is that the provision of anesthesia has become standard practice for colonoscopies and that eliminating cost sharing may increase the rates of these examinations. We examined the costs and potential benefit of the Medicare rule change.

Utilization of anesthesia services during outpatient endoscopies and colonoscopies and associated spending in 2003-2009.
CONTEXT The frequency with which anesthesiologists or nurse anesthetists provide sedation for gastrointestinal endoscopies, especially for low-risk patients, is poorly understood and controversial. OBJECTIVE To quantify temporal comparisons and regional variation in the use of and payment for gastroenterology anesthesia services. DESIGN, SETTING, AND PATIENTS A retrospective analysis of cla...
متن کاملColonoscopy without sedation: Patient factors alone are less likely to influence its uptake.
BACKGROUND AND STUDY AIMS Conscious sedation during colonoscopy minimizes discomfort, improves polyp detection rates, and reduces technical failure, but carries medication-related risks and requires dedicated and costly recovery services. Sedation-free procedures may offer a safer alternative. We aimed to compare this group with those receiving sedation to determine differences in patient chara...
متن کاملSedation with etomidate-fentanyl versus propofol-fentanyl in colonoscopies: A prospective randomized study
Abstract Background: The combination of propofol-fentanyl for sedation during colonoscopy is characterized by the frequent incidence of side effects. Etomidate-fentanyl provides fewer hemodynamic and respiratory complications. The aim of our study was to compare the safety and efficacy of propofol-fentanyl and etomidate-fentanyl for conscious sedation in elective colonoscopy. Methods: This do...
متن کاملChoice of sedation and its impact on adenoma detection rate in screening colonoscopies
BACKGROUND Studies have demonstrated that the use of sedation (regardless of type) increases polyp detection rates during colonoscopy. Compared to conscious sedation (CS), propofol sedation (PS) has led to detection of more advanced polyps, yet no apparent difference was found in the overall adenoma detection rate (ADR) in patients undergoing colonoscopy for various reasons. We aimed to assess ...
